Who are you arranging care for?

For families & caregivers

Someone you love needs care at home.

If someone you care for has a wound that isn’t healing, a new problem, or needs regular medical care but has trouble getting to an office, we can come to them. Family members, caregivers, and facility staff may contact our office directly to ask questions or request an evaluation.

In many cases a provider referral is required depending on the patient’s insurance — our team can help coordinate it.

How partnering with Revitalize works

No. 03For caregivers

Preventing pressure ulcers at home.

Pressure ulcers, also known as bed sores, can develop when a person remains in the same position for extended periods of time. These wounds most commonly affect individuals who are bedridden, wheelchair-bound, or have limited mobility.

To help prevent pressure ulcers at home, caregivers should reposition the patient every two hours, keep the skin clean and dry, and use supportive cushions or pressure-relieving mattresses when possible. It is also important to check the skin daily for early signs of redness, discoloration, or irritation.

Proper nutrition and hydration also play an important role in maintaining healthy skin and supporting the body’s ability to heal.

If you notice any open areas, persistent redness, swelling, or drainage, it is important to seek medical evaluation promptly. Early treatment can prevent small skin injuries from becoming serious wounds.
